INVITATION: Together we need to re-think how we deliver health and social care services

Julia Stackhouse (NHS SOUTH, CENTRAL AND WEST COMMISSIONING SUPPORT UNIT) has the following invitation:

“Dear All

I am writing to invite you to join me and the Transformation Programme board at a Stakeholder Engagement Event, on Monday 6 June 2016.

 The purpose of this event is to share our initial thoughts and seek feedback on our five-year Sustainability and Transformation plan with our health, social care, voluntary and community sector stakeholders.

 The areas we will be looking at are:

1.     Maternity and Paediatrics:                          Care provision for women planning to have children and expecting mothers from pre-conception, antenatal, birth and post-natal.
Care provision for children (below age 18) from universal prevention, primary and urgent care, more targeted community care and
protection for vulnerable children to specialist acute and tertiary care

2.     Urgent and Emergency Care:                       Mostly healthy: people under 75 who are mainly healthy, but may require urgent care from time to time.

People with long term conditions

Frail, mostly elderly: The integrated provision of prevention, self-care, community, primary and ambulatory care are important for people with long term conditions and frail patients to avoid emergency admissions.

3.      Elective, Diagnostic and specialist Care:     Care provision for people who require planned, routine or specialist medical or surgical services from self-care, self-assessment, diagnostics, consultation, treatments to rehab and follow up.

4.      Mental Health

5.      Learning Disabilities

Together we need to re-think how we deliver health and social care services to improve the overall health of Oxfordshire’s population, to prevent people from getting ill, to improve quality and to make best use of the budgets we have.

We very much hope you will join us. The day will include presentations on our transformation plans, but the majority of the time will be spent in discussion and seeking your views, insight and feedback.
