Dear Family Member
I am writing to provide you with an update, following my letter last month about learning disability services in Oxfordshire.
I’m pleased to be able to confirm that we are on track to complete the transfer of community adult learning disability health services from Southern Health to Oxford Health by 1 July 2017.
Our focus over the next six weeks will be on delivering a seamless transfer of services, with the vast majority of staff moving from Southern Health to Oxford Health. This means that the professionals you currently know will continue to work with your family both during and after the move.
As part of the transition process, each person’s clinical records need to be safely and securely transferred to Oxford Health. If you or your family member has any concerns about this, please contact your local Southern Health team leader to discuss this further. Records will begin to be confidentially transferred in June, so please contact them soon if you have any queries about this process.
While the location of your local learning disability team will not change on 1 July, their contact telephone numbers and emails will change. From 1 July, the contact details for the teams will be:
Jubilee House 5510 John Smith Drive Oxford Business Park South Cowley Oxford OX4 2LH Telephone: 01865 336800 Email: firstname.lastname@example.org
