Job description:
Main Duties and Responsibilities

1. Service delivery

1.1. Use an agreed framework to do an initial assessment with a patient of their community and social needs, and develop a plan with them to improve their health and wellbeing. This will also consider the impact of the patients’ family / household on their health and wellbeing

1.2. Develop an exit strategy for the patient which ensures that; the change is sustainable and that they know how to access future support and are linked into community networks

1.3. Ensure verbal consent from patients for any telephonic assessment, and ensure written consent paperwork is in place and recorded following the initial visit

1.4. Promote a culture based on the principles of person centred support, ensuring dignity, choice, respect, independence and rights are upheld at all times

1.5. Identify, report and monitor any safeguarding or vulnerable adult risks and concerns

1.6. Work within the framework of the Citizens Advice North Oxfordshire Lone Working policy when working one-to-one with patients, offsite or on home visits.

2. Service Development

2.1. In liaison with the Social Prescribing Manager and GP Social Prescribing Leads, contribute to the strategic planning and development of the service, and develop and review operational plans for the effective delivery of the service

2.2. As one of the social prescribing team, develop and maintain quality assurance and improvement systems that monitor standards and support high quality service delivery

2.3. To ensure that community providers to which people are referred meet quality assurance standards

2.4. Work proactively with health care professionals to stimulate referrals in line with agreed key performance indicators

2.5. Provide timely feedback to referral partners on the support provided to patients as required

2.6. Support project development by raising awareness of the service through training, presentations and events.

3. Monitoring, Reporting and Evaluation Tasks

3.1. To maintain electronic records of all patient activity and project tasks

3.2. Support the Social Prescribing Manager to meet targets and deadlines and ensure compliance with funding requirements for the social prescribing service

3.3. To monitor the performance of the service against targets and produce reports on the progress and plans for the Social Prescribing Manager

4. Training and Development

4.1. Attend one-to-one meetings and annual performance appraisal meetings with the Social Prescribing Manager

4.2. Undertake regular training for the development of the service and individual career progression

5. Other responsibilities

5.1. To work flexibly to meet the needs of Citizens Advice North Oxfordshire, including some occasional evening or weekend work, and to travel within Oxfordshire

5.2. To carry out any other duties appropriate to the post

5.3. The post holder must keep their work within the stated aims and objectives of Citizens Advice North Oxfordshire and its policies at all times
