Role summary: Working within local communities across Oxfordshire you will be joining our successful Community Information Network Team as we work to further improve access to information, support, activities and opportunities for all those with a health, care or support need as well as supporting those we work with to access the digital world and build their confidence in doing so. You will be working with individuals, families and groups both in the community and in their own homes as well as a network of local digital champion volunteers. We take a blended approach so you will be involved in different project streams within your role which might include working within the hospital setting, alongside adult social care or within GP surgeries but all are underpinned by our work with local communities to further build upon and support what is being offered as well as working with individuals and their families to connect them with the support and opportunities that exist, particularly with a focus on the digital world. We are passionate about supporting and empowering people so that they can maintain their independence, health, wellbeing and ‘live life to the full’.

We have contracts available across the county for 21 to 35 hours a week. minimum of 3 working days.

This is a 12 month initial contract with the potential to extend.

MAIN DUTIES:

1. Ensure that local resources within communities are mapped and work to further develop and sustain the network of grassroots partnerships with agencies, individuals and groups working in the locality, with a particular focus on digital activities and support.

2. Taking a strengths based approach you will support individuals and their families by helping them to identify needs and desired outcomes. Providing face to face information, signposting and training in both community and hospital settings and people’s own homes, supporting them to create an individual plan as appropriate; Keep information and advice knowledge up to date by attending regular training sessions and team meetings

3. By being responsive and creative, develop, promote and deliver an increasing menu of activities and opportunities for older people within their community that are sustainable and have a digitally based focus. Help improve access to available services, supporting people to both know about and link into the support that exists through a range of community based network marketing events, information fairs, drop-in’s and talks

4. Work with, further develop and sustain the community link/digital champion volunteer network, building and supporting a team of digital volunteers to help run activities, classes and support individuals to become more confident and proficient in all things digital.

5. Ensure that systems are in place for receiving and recording regular feedback from people using the service whilst maintaining accurate and up to date records re work done. Contributing towards the effective monitoring and review of the contract(s)
