Healthwatch Oxfordshire: Share your views – what does ‘joined up’ care mean to you?

July 11, 2023

Share your views – what does ‘joined up’ care mean to you?

Health and care services are working to become more integrated in the care they provide for patients.

This means care from health professionals and across different services will be more ‘joined up’ around people’s needs.

Healthwatch Oxfordshire would like to hear from you about your experience of ‘joined up’ care.

Please share your thoughts by completing this short, anonymous survey

Your views will help shape the support people need and to improve services as Healthwatch Oxfordshire will share your feedback with decision-makers.

If you would like a copy of this survey in another format or language, or would prefer to speak to someone about this, please contact Healthwatch Oxfordshire on 01865 520520 or at hello@healthwatchoxfordshire.co.uk

Please note this survey will close on Friday 28th July.
